Run each query against reporting_db, save the results as prov_name.csv, prov_npi.csv, and service_loc_enroll.csv. Query 4 (plan_networks.csv) is optional — only needed when seeding new enrollments via the "Type a list" mode in Step 3.

Map roster columns onto the template

The 24-column template above is the contract. The fields below tell the engine which roster columns fill the user-controlled cells; the rest auto-populate from prov_name.csv + service_loc_enroll.csv via match.

Sheet(s) Ctrl/⌘-click to select multiple sheets. All selected sheets must share the same header layout (row offset + column names) and are concatenated before matching.

First name (only if separate column) leave blank if Name is "Last, First" in one cell

Plan networks & effective date

Drives roster_plan_network and enroll_dt on every output row.

A column on each row.

Each roster row has a Program/LOB column and an Effective Date column.

Spread across multiple columns.

Each plan network is its own column header; the cell value is the effective date.

Type a list — every provider gets the same set.

Roster has no PN columns; the rule lives outside the file. The tool synthesizes columns in memory.
